- Description: Your body raises a blocking hormone within hours of a dose. Iron-depleted women absorbed considerably more by taking it every other day instead.

| Restless legs is the complaint I hear about most and see written about least. That crawling, fidgeting need to kick the sheets loose the moment you finally lie still, worse in the evening, worse again the longer you stay put. Plenty of women already know iron is involved and have been dutifully swallowing a tablet with breakfast for months. Then the legs carry on exactly as before, and the conclusion drawn is that iron was another dead end. The tablet was probably right. Instructions on the back of the box were not. Nothing was wrong with your absorption. You were dosing in a pattern that switches absorption off. Every iron bottle on that shelf says take one daily. The research says daily is close to the worst way to do it. THE HORMONE THAT SHUTS THE DOOR Iron is useful and iron is also dangerous in excess, so your body guards the gate carefully. A hormone called **hepcidin** controls that gate, and a dose of iron sends it climbing within a couple of hours. Raised hepcidin stays raised for roughly a day. Swallow the next tablet the following morning and it arrives at a gate that is still half shut, so most of what you paid for travels straight through you. Skipping a day lets the gate open again, and the smaller schedule delivers the bigger dose. THE EVIDENCE Diego Moretti and colleagues at ETH Zurich tracked absorption in iron-depleted young women given supplements on different schedules (*Blood*, 2015). Doses taken **daily or twice daily drove hepcidin up and fractional absorption down**. Spacing the same iron across alternate days let more of every dose actually cross into the body. Separately, sleep medicine sets the ferritin threshold for treating restless legs at 75, while most lab reports call 30 perfectly normal. Two numbers, then. One decides whether you need iron at all, and the other decides whether any of it lands. IN ORDER OF EFFORT TEST BEFORE YOU TAKE Ask for ferritin by name, and never supplement iron blind. A standard blood count does not include it, so you have to request the word itself. This step is not optional politeness. Some people store iron far too easily, and loading up without knowing your number does real harm, which is the one thing the supplement aisle will never print on a box. THE ALTERNATE-DAY SWITCH One dose, every other day, instead of one every morning. Half as many tablets, more iron absorbed, and considerably less of the constipation that makes most women abandon it by week three. Anybody who has quit iron because of what it did to their stomach should know the schedule was doing much of that. THE VITAMIN C PAIRING Swallow it with vitamin C, and nowhere near your tea. The trials behind this paired every dose with about 100 mg of it, which keeps iron in the form your gut can take up. Tea, coffee, milk and calcium tablets all drag absorption the other way, so the morning cuppa alongside has been undoing the whole exercise. THE 12-WEEK RETEST Book the follow-up blood test when you start, not later. Ferritin climbs slowly and legs settle gradually, so three months is the honest window before judging any of it. Booking the retest on day one also stops the other failure mode, which is carrying on indefinitely long after your stores have refilled. And one more thing... Some stomachs simply will not tolerate ferrous sulfate whatever the schedule. There is a gentler route that almost nobody in the supplement aisle talks about. **Lactoferrin** is a protein found in milk and in human colostrum, and it carries iron across the gut wall by a different door, appearing to sidestep much of the nausea. It is stocked for immunity, beside the elderberry, at four times the price it ought to be. Filed under colds, when its most interesting job involves iron.
